At FTL Logistics, we rely on the professionals who haul our loads to help build and maintain our customer relationships. That's why we expect any carrier interested in FTL Logistics to meet the same strict criteria and high standards of service that we demand of our owner operator fleet.
The following information is needed to set up your company with FTL as an approved carrier.
You may submit all of the items below using our NEW Online Carrier Application Tool !!! Try it !!!
If you do not use our Online Carrier Application Tool , please fax items below:
- A signed copy of our brokerage agreement -- see our Logistics Application
- A copy of your operating authority certificate (Not necessary if Online Application is submitted)
- A cargo insurance certificate for a minimum of $100,000 naming FTL Logistics, LLC. as additionally insured.
- A BLANK COPY IS NOT SUFFICIENT. You must have your insurance company fax and mail us a copy.
- A auto liability insurance certificate for a minimum of $1,000,000 naming FTL Logistics, LLC as additionally insured.
- A BLANK COPY IS NOT SUFFICIENT. You must have your insurance company fax or email us a copy to Insurance@goftl.com (Not necessary if Online Application is submitted)
- Worker's Compensation insurance (if applicable)
- A completed W-9 form
- Your D.O.T. rating (if applicable)
